Starting Jan. 1, Marquette University is offering a chronic condition management program, administered by Tria Health. Eligibility for this program will be limited to employees, spouses and their dependent children who are enrolled in one of Marquette’s medical plans and have been diagnosed and are being treated for a chronic health condition.
Examples of chronic conditions Tria Health will help members manage include diabetes, heart disease, high cholesterol, high blood pressure, mental health disorders, asthma/COPD, osteoporosis and migraines.
Tria Health’s pharmacists are available to support you in managing your health. Not only will they make sure your medications are working as intended and that you’re on the right medications, but they can support you by creating a care plan that is specific to your needs and share it with your doctor.
Tria Health will contact eligible members directly through a welcome letter and will follow up by phone and email with more details on the programs. Tria Health determines members eligibility based on claims data they receive directly from UMR and Navitus.
