Complete biometric screening and Health Risk Assessment

Biometric Screenings
Biometric screenings began on Monday, Oct. 28. By completing a free biometric screening before Nov. 16, users will earn 4,500 points toward their 2020 FSA/HSA contribution.

There are three ways to complete the biometric screening:

  1. Schedule a 15-minute Aurora Health Care screening appointment online or call 1(877) 765-3213. Appointments are available:
    • On campus in the AMU and the Dental School, Monday through Saturday
    • Off campus on Saturdays
  2. Fax lab results completed between July 1 and Nov. 16 to Aurora Health Care at (414) 525-2570 by noon on Nov. 16 using the Primary Care Provider Form. The discloser form MUST be completed and faxed to Aurora.
  3. Complete a comparable biometric screening at a spouse’s employer-sponsored screening event and fax the results to Aurora Health Care at (414) 525-2570 by noon on Nov. 16. The discloser form MUST be completed and faxed to Aurora.

Online Health Risk Assessment
Users can take the online Health Risk Assessment through via the Virgin Pulse wellness portal until Nov. 19 at 4 p.m. Upon completion, users will receive 2,500 points toward the 2020 FSA/HSA contribution.
