As part of Human Resource’s ongoing annual compliance efforts and in an attempt to cut down on the amount of printed materials sent out, all required health and welfare plan notices will be posted on the benefits website on the Articles & Resources tab. Below is a brief description of each notice that has been added or revised.
Summary Annual Reports
Reports for the Jan. 1, 2018, through Dec. 31, 2018. plan year summarize basic financial information contained in the annual reports, which are filed with the Employee Benefits Security Administration as required under the Employee Retirement Income Security Act of 1974 (ERISA).

These reports pertain to individuals covered under any of those plans.
Summary Annual Report
Reports for the Sept. 1, 2016, through Aug. 31, 2017, plan year summarize basic financial information contained in the annual report, which is filed with the Employee Benefits Security Administration as required under ERISA.

The Medicare Part D Creditable Coverage Disclosure Notice pertains to individuals who are covered under the Marquette University group health plan and who are eligible to enroll in Medicare in 2020. It contains important information you need to know about the coverage offered through Marquette and Medicare’s prescription drug coverage.
The Premium Assistance Under Medicaid and the Children’s Health Insurance Program (CHIP) Notice provides information on states that may have premium assistance programs that can help pay for employer-sponsored coverage using funds from their Medicaid or CHIP programs.
Important Enrollment Notices for group health plan participants includes a Notice of Availability of Privacy Practices Notice, Notice of Special Enrollment Rights, Annual Notice of Women’s Health Rights and a Notice of Non-Discrimination and Accessibility Requirements.
Notice Regarding Wellness Program provides information regarding the wellness program and outlines the protections associated with how personally identifiable health information is disclosed and maintained.
The Marketplace Notice provides basic information regarding the health insurance Marketplace and employment based health coverage offered by Marquette University.
