Summary Annual Reports for the 2017 plan year (Jan. 1 – Dec. 31) summarize the basic financial information contained in the annual reports filed with the Employee Benefits Security Administration as required under the Employee Retirement Income Security Act of 1974 (ERISA).

These reports pertain to individuals covered under any of those plans.
The Medicare Part D Creditable Coverage Disclosure Notice pertains to individuals covered under the Marquette University group health plan and eligible to enroll in Medicare in 2019. It contains important information needed about the coverage offered through Marquette and Medicare’s prescription drug coverage.
The Premium Assistance Under Medicaid and the Children’s Health Insurance Program (CHIP) notice provides information on states that may have premium assistance programs that can help pay for employer-sponsored coverage using funds from their Medicaid or CHIP programs.
Important Enrollment Notices for group health plan participants includes a Notice of Availability of Privacy Practices Notice, Notice of Special Enrollment Rights, Annual Notice of Women’s Health Rights, and a Notice of Non-Discrimination and Accessibility Requirements.
Notice Regarding Wellness Program provides information regarding the wellness program and outlines the protections associated with how personally identifiable health information is disclosed and maintained.
