HRA online health assessment deadline is Nov. 22

Access to the Health Risk Assessment online health assessment is still available to all employees and their plan-enrolled spouses through Wednesday, Nov. 22. Medical plan participants who complete both HRA parts, the biometric screening and the online health assessment, will receive the 2018 incentive. All information regarding the HRA process is available on the benefits website.

  • Employees must complete their online health assessment by the end of the day, Wednesday, Nov. 22
  • Employees only: Your username is your 9-digit Marquette University Identification (MUID) number followed by an uppercase E.  If your MUID number has fewer than nine digits, add leading zeros to create a nine digit MUID. For example, if your MUID number is 123456, your username will be 000123456E.
  • Spouse only: If you are not employed by Marquette University your spouse’s username is your spouse’s employee username (9-digit Marquette University Identification Number) followed by an uppercase S and your 8-digit date of birth displayed as MMDDYYYY. For example, the spouse username will be 000123456S11121970.  *If you are employed by Marquette, use your 9-digit Marquette University Identification (MUID) number followed by an uppercase E.
  • Initial password: Your initial password is MRQ followed by your 8-digit date of birth displayed as MMDDYYYY.

Passwords are case-sensitive, and should be a combination of letters and numbers, no special characters. If you need assistance logging in, click the “Need help logging in?” link for additional details, or contact Preventure Customer Support at (888) 321-4326. Be sure to bookmark or favorite the site.

Next time you login, enter your username and password in the fields on the login page and click “Sign in”.

To verify that Preventure received your biometric screening results and your online health assessment was completed, you can log into the Preventure Wellness Portal and make sure you have earned 100 Golden Eagle Points.  Alternatively, you can contact Preventure Customer Support.

There will be no exceptions. Wednesday, Nov. 22, is the final date to complete the HRA online health assessment, using your biometric screening results. Once the date has passed, you will have missed your opportunity for the 2018 incentive.
