Required HR notices

As part of our ongoing annual compliance efforts and in an attempt to cut down on the amount of printed materials sent out, we are posting all required health and welfare plan notices on our benefits website at http://www.marquette.edu/hr/employeebenefits.shtml (on the Articles & Resources tab). Below is a brief description of each notice that has been added or revised.

Summary Annual Reports for the 1/1/2016 through 12/31/2016 plan year summarize the basic financial information contained in the annual reports filed with the Employee Benefits Security Administration as required under the Employee Retirement Income Security Act of 1974 (ERISA).

  • Marquette University Group Life Insurance Plan (Plan 502)
  • Marquette University Flexible Benefit Program (Plan 510)
  • Marquette University Group Legal Plan (Plan 511)
  • Marquette University Group LTD Plan (Plan 503)

These reports pertain to individuals covered under any of those plans.

The Medicare Part D Creditable Coverage Disclosure Notice pertains to individuals covered under the Marquette University group health plan and eligible to enroll in Medicare in 2018. It contains important information you need to know about the coverage offered through Marquette and Medicare’s prescription drug coverage.

The Premium Assistance Under Medicaid and the Children’s Health Insurance Program (CHIP) notice provides information on states that may have premium assistance programs that can help pay for employer-sponsored coverage using funds from their Medicaid or CHIP programs.

Important Enrollment Notices for group health plan participants includes a Notice of Availability of Privacy Practices Notice, Notice of Special Enrollment Rights, Annual Notice of Women’s Health Rights,   Notice of Non-Discrimination and Accessibility Requirements and a Health Insurance Marketplace Notification.

Notice Regarding Wellness Program provides information regarding the wellness program and outlines the protections associated with how personally identifiable health information is disclosed and maintained.
