Participation in the Health Risk Assessment process includes a biometric screening and completion of an online health assessment questionnaire after the screening. The employee and enrolled spouse must complete both parts of the HRA process in order to receive the 2018 incentive. If only one of the two complete the required steps, the employee will not be eligible for the incentive. The incentive for 2018 will be outlined in the Benefits Enrollment Guide mailing that is being sent to homes the week of October 9.
The first step, the biometric screening, can be completed free of charge on-campus and at designated off-site Aurora locations for the employee and their spouse. Online scheduling for the biometric screening portion of the HRA process begins today for appointments starting on Monday, Oct. 23.
To accommodate the number of people for these appointments, early morning and evening options have been added to the on-campus offerings. In addition, Saturday morning biometric screening appointments will be available on campus and at designated off-site Aurora locations for the convenience of spouses to complete this step. Employees are encouraged to participate in the on-campus offerings during the week as there are a limited number of appointments available during these extended offerings.
Biometric screenings may also be completed at a primary care provider utilizing the Primary Provider Form available on the HRA section of the Benefits website or by completing a comparable biometric screening at a spouse’s employer-sponsored HRA screening. Biometric screenings for employees and their spouse must be completed between July 1, 2017 and Nov. 17, 2017.
